All referrals to residential services are required to come from CLCM to each provider.

Comprehensive 24 hour residential services:


These services consist of a variety of provider organizations that operate homes within the community. All homes are staffed 24 hours per day with staff to support individual care and support needs.  Although programs vary, services will include medical, behavioral social and personal care needs.  All homes are licensed by the State of Oregon and services provided are monitored by CLCM service coordinators.

Support Living Services:


These residential services include supports in a family home or a persons own home/apartment.  Services are not 24 hour per day, but do offer supports in money management, medical needs and medical management, community integration, personal care and daily living skills.
